Data Confirmation

Each year, families complete the Aeries Data Confirmation process. Beyond updating emergency contacts and student details, completing this form ensures Fountain Valley School District (FVSD) receives funding for essential student resources—including after-school programs, school meals, and academic support services.

Online Data Confirmation replaces traditional paper forms. All information submitted through the Aeries portal is secure and protected.

Completing Data Confirmation on Mobile

Step-by-Step Guide

Parent Data Confirmation consists of seven steps. Use the guide below to complete each section.

Step 1: Family Information

Step 2: Income Survey

Under the California Universal Meals Act, public schools provide two free meals every school day to all transitional kindergarten through 12th-grade students, regardless of household income. This eliminates the need for a traditional meal application and reduces paperwork for families.

However, state and federal funding for school resources is still calculated based on household income eligibility. The Income Survey is a simple alternative to the traditional meal application that ensures FVSD receives the full funding required to continue offering free meals and other student programs.

Questions asked in this section include:

How many people live in your household?
What is your total monthly household income?

Why your response matters: Completing this brief survey helps Fountain Valley School District secure crucial funding for teachers, counseling, technology, meals, and academic support services across all schools.

Note: Your income information is strictly confidential and is used solely to determine district funding eligibility.

Step 3: Contact Information

The Contacts screen allows you to update information for parents, guardians, and emergency contacts to ensure your school site has accurate records.

Parent/Guardian Requirements:
Each student must have at least one contact designated as a Parent or Guardian. This contact requires the following details:

Full Name
Phone Number
Email Address
Relationship to Student
Education Level
Notification Preference (Select "General and Emergency" for general school announcements, event notifications, and attendance calls)

Emergency Contact Requirements:
For additional emergency contacts, provide:

Full Name
Relationship to Student
At least one valid Phone Number
Notification Preference (Select "Emergency Only")

Note: Contacts designated as "Emergency Only" will only be reached during urgent situations requiring immediate contact.

Step 4: Medical History

The Medical History section allows you to review, edit, or add medical conditions for your child to ensure school health personnel have accurate health records.

To complete this section:

Review existing medical conditions listed for your child.
Select additional conditions from the predetermined dropdown list if applicable.
If your child has no medical conditions to report, select the option indicating no medical history to report and continue to the next step.

Step 5: Insurance Information

Insurance information will be used in case of a student emergency, field trips, athletics and potential billing for mental and behavioral health services. FVSD is hoping to streamline this information to eliminate the need for parents to provide information multiple times throughout the school year. Insurance will not be shared or used unless you provide explicit consent.

Click on the +Add Record button in the middle of the screen to open up the insurance entry window.

In the pop-up window, select your insurance type: Medical, No Insurance, or Decline to State.

Add the effective date of your Insurance Policy (Date you started coverage), or if you chose No Insurance or Decline to State, put today's date. Do not put any information in the Expiration Date Field.

Then, under Insurance Carrier, search for and select your Insurance Provider. For No Insurance or Decline to State, search for and select "None" from the dropdown.

For those adding medical insurance information, please complete all remaining fields in the Add Record window.

Additional required fields include:

Member ID
Subscriber First Name, Last Name, & Address

Once completed, click the Save button in the lower right corner.

Please wait for the insurance submission to load on the page. Then, click “Confirm and Continue.”

Step 6: Documents

The Documents section provides essential annual school information, rights, and policies. Documents provided in this section may include:

Student and Parent Handbooks
Student Rights and Responsibilities
Health and Medical Policies
District Information and Resources

How to complete this section:

Select each document link to open and review it. (Note: Links will open in a new browser tab or window.)
Once opened, the system automatically registers that the document has been reviewed.
Confirm that all required documents show as read to unlock the next step.

Step 7: Authorizations

The Authorizations section allows you to grant or withhold consent for specific school disclosures, such as media releases for district social media, website features, or news publications.

To complete this section:

Review each authorization prompt carefully.
Select Allow or Deny (or Yes / No) for each item listed.
Save your responses to proceed to the final step.

Note: Fountain Valley School District does not release student records or personal information to non-educational organizations or individuals without explicit parent or guardian consent.

Step 7: E-Signature and Final Submission

The final step confirms that all information provided during the Data Confirmation process is complete and accurate.

To complete your submission:

Review the summary on screen to verify that your entries are correct.
Enter your digital signature where prompted and select Save and Continue (or Finish and Submit).
After submitting, locate the message that reads: "Thank you for confirming the student data in the system."
Select the Print New Emergency Card button to generate your student's official emergency card.

Important Action Required: Please print and bring the signed Emergency Card to your school site on the first day of school, during registration, or at Back-to-School Night.

Common Tech Fixes & Troubleshooting

Data Confirmation Tab or Alert is Missing

Problem: You just logged in or added a student, but you do not see the Data Confirmation alert banner or tab.

Solution:

Log out of your Aeries account, then log back in.
If the alert still does not appear, use the top menu to navigate to: Student Info > Data Confirmation.
If you receive a "System Error" message or nothing happens, contact your school site or district office to verify and update your Parent Portal permissions.

Missing Children in Your Account

Problem: Not all of your enrolled children are visible in your portal account.

Solution:

Look at the upper-right corner of the desktop portal and select your email address.
Select Add New Student to Your Account from the drop-down menu.
Enter the Verification Code provided by your student's school site to link their account.

Error: "No Parent/Guardian Contact Record Associated"

Problem: Aeries prevents you from completing the Contacts section, stating you are not tied to the student record.

Solution: This issue usually occurs if the school has your contact record listed as a general contact rather than a primary Parent/Guardian, or if your portal email address does not match school records exactly.

To resolve this, contact your school site's front office so a staff member can update your contact classification in the system.
